What is an RMMR?

A Residential Medication Management Review (RMMR) is a structured, collaborative review of a resident’s medicines, conducted by a credentialed pharmacist in partnership with the resident’s GP and facility team.

RMMRs are designed to identify, prevent and resolve medication-related issues in older adults living in residential aged care facilities (RACFs). The service is Medicare-funded and forms a key part of the Aged Care quality standards and medication safety in residential settings.

An HMR is for people living in the community, while an RMMR is for residents of aged care facilities. Medicare funds both and involve accredited pharmacists working with GPs to review medications.

Only a general practitioner (GP) can refer a resident for an RMMR. Referrals must meet Medicare criteria.

A resident is typically eligible for one RMMR every 12 months, unless earlier review is clinically justified e.g. recent hospitalisation, medication changes, deterioration.
